November 3, 2025 It has been my experience that many medical providers have been performing injections into joints and clinical results vary. Prior to ultrasound guided injections, medical providers training for joint injections included learning the landmarks by palpation for needle insertion and treatment. While this was standard of care for many years, the results vary. From procedural pain, proper needle placement there were difficulties as there was not visual guidance once the needle entered the body. Clinical results varied greatly based on going blind into the joint. Numerous studies have shown that when an ultrasound is used to visually guide the needle to the target the patient results are significantly better. And is that not what we all want? Great patient outcomes! Ultra sound guided injections for placement of the needle is easy to learn and provides a significantly higher level of precision for proper needle placement than using palpation and landmarks only techniques. Ultrasound is safe, providers have improved precision with needle placement, and is easy to use. Many patients are now becoming more familiar with the use of Ultrasound guidance for injections and they are wanting to go to trained providers as this will improve their experience, less pain, and improve outcomes. When looking at studies, the data strongly supports visual guidance with joint injections for improved patient results, less pain, improved patient outcomes, and overall better experience for the patient. Lee et al reports that the accuracy with ultrasound guidance in the shoulder goes from 72% to 92.5% resulting in increased range of motion, improved shoulder function, and improved pain intensity within two weeks of the injections. A randomized control trial evaluating the effectiveness of visually guided injections for treatment of OA in the knees concluded that there was 48% decrease in procedural pain, 52% decrease in non responder rate, 36% increase in therapeutic duration, and 58% reduction in cost per responder per year (Sibbitt, et al, 2011). Studies support improved accuracy, safety, and decreased procedural pain with ultrasound guided injections into musculoskeletal conditions. As medical providers, we all got into healthcare to help our patients. We know once we learn more, we do more. After 20 years in healthcare working in many settings, I can see the training and operating basis of traditional western medicine and integrated physical medicine clinics vary widely. The medical department consisted of one person (me) and my office was also my treatment room. In the Downtown’s Healthcare facility, we have two NPs, two MAs, four medical treatment rooms, one IV treatment room, and an aesthetics room. Here are just a few keys to this expansion. First and foremost DHC has a tremendous purpose. This purpose has been shared with the team over and over. I bought into this purpose quickly after starting. I fully believe that the healthcare system in this country is broken. I fully agree that patients should be treated from a more naturalistic stand point. I am blessed everyday to take care of patients and help them get off drugs and avoid unnecessary surgeries. Secondly, we focus on the following on a continual basis: • We Train everyday! • We added multiple medical rooms • We have solid systems in place • We have great verbal and nonverbal communication systems in place • We operate of a set of core values that the team is managed by • We continue to update our policies to improve our delivery and patient care flows • Etc… So, the question is, How do I serve more patients? The above list is a starting point, however, the number one key as I discussed is purpose! What is your why? Once the owner of the organization has this, do they spend the time relaying this purpose? Leadership starts at the top! I’ve been fortunate to be on a team that has this leadership. We spend time every week reviewing this purpose and everyone in our office operates off their personal, professional and financial goals. The medical provider must focus on purpose and core values of the office. Our purpose at Downtown’s Healthcare is to get people off of medications and avoid surgery. Coming from this perspective with a conservative approach it is easy to communicate with the patient through the fear of needles and help them get the treatment they need. The patient came to the office for a reason. It is often times something that has impacted them enough that they decided to finally take action. Knowing this reason of why they took action is a key ingredient for the medical provider when communicating with the patient. For example, when a grandparent is not able to play with grandkids and it is impacting their quality of life. Communicating with the patient in a way they can experience it while getting into agreement is key to moving through the fear of needles and improving the patient function and quality of life! Techniques we use to minimize fear, worry, and make it be a great experience to receive treatment. Fear of needles and injections is the number one objection I hear everyday from patients, staff, and medical providers. I often hear “do I really need this?” My answer is “yes!” We take a conservative approach with medications for these injections. Part of the integrated physical medicine model includes injections into muscles, joints, and nerve blocks. The syringes can include a variety of treatment options. In our office, we focus on getting people off of unnecessary medications and avoiding unnecessary surgery. Some potential options for treatment with specific conditions include numbing agents (Lidocaine or Marcaine), anti inflammatory agents (Sarapin, Traumeel, Ketorolac). Regenerative options may include PRP (Platelet rich plasma), allograft tissue, bone marrow transplant. It is important to communicate with patients the purpose of taking a conservative approach in the integrated medical setting. July 6, 2025 Shoulder joint/capsule: 72% to 92.5% accuracy w U/S (Lee et al). The improvements in pain intensity, range of motion, and shoulder function score were significantly greater in the ultrasound-guided injection group than in the landmark-guided injection group by the second week after injection (P < .05). Bicep tendon: Hashiuchi et al20 studied 30 patients with reported anterior shoulder pain with a primary diagnosis of tenosynovitis, tendinitis of the biceps tendon, or both. Shoulders were randomly allocated into ultrasound-guided and landmark-guided injection groups. Computed tomography (CT) imaging was performed immediately after a contrast agent was injected into the biceps tendon sheath. Ultrasound guidance resulted in 86.7% of injections into the tendon sheath, whereas landmark guidance resulted in 26.7% into the tendon sheath, which was a significant difference (P < .05). Wrist and hand: Goncalves et al18 conducted a case series demonstrating that ultrasound-guided injections of the metacarpophalangeal joint were efficacious and visual analog scale scores were improved. Raza et al34 assessed the accuracy of proximal interphalangeal (PIP) and metacarpophalangeal (MCP) joint injections: Palpation-guided injections were intra-articular 59% of the time (6/12 PIP and 4/5 MCP joints) compared with 96% (24/26 PIP and 27/27 MCP joints) for ultrasound guidance. Hip Yoong et al53 evaluated the utility of ultrasound-guided hip joint injection in predicting surgical outcomes. In that study, 138 patients underwent ultrasound-guided injection with local anesthetic to aid in diagnosis of hip disease prior to surgical hip arthroscopy. Yoong et al53 found that 93% of patients who had pain relief after injection had successful surgical outcomes, compared with 63% of patients with partial relief; 44 of 49 patients who had no relief from the injection ultimately did not have hip surgery. Knee Curtiss et al,10 in a cadaveric ultrasound-guided versus landmark-guided study published in 2011, showed a range of accuracy of 55% to 100% in the landmark-guided group. The results for landmark-guided injections were dependent upon the injector, while the ultrasound-guided injections were 100% accurate regardless of injector.
